Reinstatement of Certificate of Registration
Registrants who have been suspended for non-payment of registration fees cannot practise dental hygiene in Ontario. To reinstate the Certificate of Registration, an online Application for Reinstatement must be completed in the Self-Service Portal.
If the registrant is taking some time away from practice, they might choose the online Application for Reinstatement - Inactive Certificate of Registration. CDHO registrants with Inactive Certificates of Registration cannot practise dental hygiene in Ontario. The signed form and payment must be submitted to the College for processing.
If the registrant is planning on returning to practise dental hygiene in Ontario, choose the online Application for Reinstatement - General/Specialty Certificate of Registration. The following documentation must be submitted:
For registrants who have practised outside of Ontario, Form B must be completed and submitted directly by the regulatory body in any jurisdiction in which they have practised
- Registrants who have not practised dental hygiene in a recognized jurisdiction within three years prior to applying, must successfully complete an approved refresher course, or assessment set or approved by the Registration Committee
- All registrants must maintain professional liability insurance which includes: a minimum of $1,000,000 coverage for each occurrence; annual aggregate coverage of no less than $5,000,000; and a deductible of no more than $4,000 per occurrence. In addition requirements include: run-off coverage for a minimum of two years; the insurer must be licensed with the Financial Services Commission of Ontario or the office of the Superintendent of Financial Institutions Canada; and a sexual abuse therapy and counselling fund endorsement that, i) Provides coverage for therapy and counselling for every person eligible for funding under subsection 85.7(4) of the Code; and ii) Provides coverage, in respect of each such eligible person, for the maximum amount of funding that may be provided for the person under the Act, for therapy and counselling as a result of sexual abuse by the Registrant.
- This coverage must be in place before being registered.

Once a completed application package has been received, it takes approximately 5 to 10 business days to process. An application is considered complete once all required documentation has been received and verified. If any information is missing or incomplete, the expected timeline will be lengthened depending on the item(s) pending for submission.
