Registrants who wish to resign from the CDHO, need to formally notify the College by completing an online Resignation Form in the self-service section. You will be notified once your resignation has been processed.
Registrants who are currently under investigation and wish to resign the certificate of registration, should send an email to firstname.lastname@example.org. A notation will be made on the public register for resignations while under investigation
Failure to renew or to formally resign on or before January 1st, results in the suspension of the Certificate of Registration for non-payment of fees. Certificates of registration are automatically revoked two (2) years after being suspended. The public register will permanently indicate that a certificate of registration is “suspended” or “revoked” and this information will also be published in the College publication, Milestones.
