If you need to update your information

On the renewal form, you are asked to review the information CDHO has on file (e.g., home address, business address, email, telephone number, etc.). Please ensure that your information on file is up to date. Outside of the renewal period, you can update your information under the “Profile Section” of the Self-Service Portal. As a reminder, if there are any changes to your personal or business information, please update your profile within 14 days of the change. Keeping your information accurate and up to date helps us to provide you with better service and communication.
