Collection and Use of Personal Information
The College of Dental Hygienists of Ontario collects personal information through the Self-Service Portal and other forms and documents completed through the registration or reinstatement process. This occurs under the general authority of the Regulated Health Professions Act, 1991, the Dental Hygiene Act, 1991 and the College’s bylaws.
Personal information is used for the purpose of assessing eligibility for registration or reinstatement, including any appeals or reviews. All documents submitted become the property of the College. Upon registration or reinstatement, the information becomes part of the registrant’s file.
The College may use all personal information collected to perform its regulatory functions under the legislation. Personal information may also be used for aggregate statistical reporting and analysis within the College and externally (e.g., reporting to the Ministry of Health). The College takes appropriate measures to safeguard the confidentiality of personal information provided.
