The renewal period begins on October 15, 2025, and RDHs have until December 1 to complete the form and pay the annual fee. Learn more about renewal.
You’ll notice the renewal form looks a little different this year. In 2024, we upgraded our database, which powers the Public Register and Self-Service Portal. New features include the ability to upload supporting documents, such as your professional liability certificate and any relevant disclosure documents. We’ve also added helpful prompts to guide you in accurately completing the form.
The way you log in to the Self-Service Portal has also changed. You now log in using the email we have on file for you, and you will be prompted to create a new password. If you haven’t logged in to the Self-Service Portal since March 2025, please follow the instructions below to set up your account before the renewal period begins:
- Go to https://selfservice.cdho.org
- Click “Forgot your password?”
- Enter the email address you registered with CDHO
- If you do not know/do not have access to your email address, please contact CDHO at registration@cdho.org for assistance. Please do not sign into the Portal with a new email address — this is only for new applicants.
- Click “Send verification code”
- Enter the verification code sent to your email address and click “Verify Code”
- Click “Continue”
- Select a new password and enter it into “New Password” and “Confirm New Password”
- Click “Continue”
Why we collect your data
The renewal form collects a range of data, including demographic, practice, and education information. Much of this information is required by the Ontario Government. The Ministry of Health collects the same information from all regulated health professionals to help with health human resource planning. Certain regulated professions (including CDHO) report annually to the Office of the Fairness Commissioner of Ontario, which works with regulated professions to ensure their registration processes are transparent, objective, impartial and fair. All data is submitted to these organizations anonymously to protect privacy.
Since 2023, we’ve collected voluntary demographic data in the application and renewal forms. These questions help us gather information along four diversity dimensions: Indigenous identity, visible minority identity, sexual orientation and gender identity, and disability. This data helps us better understand demographic trends in the profession and guides efforts to improve equity and inclusion. We only use aggregate and de-identified data for statistical purposes, such as for reports, analyses and studies, and to provide information on group representation in the profession.
