Get ready for Renewal!

CDHO’s annual renewal period begins on October 15, 2025 and RDHs have until December 1, 2025 to complete the renewal form and pay the annual registration fee. Learn more about renewal.

The updated Self-Service Portal launched in March 2025. All RDHs will need to sign in to the Portal to renew their Certificate of Registration during the renewal period. Logging in beforehand ensures that you have confirmed your new credentials and that any technical issues can be resolved.

If you already logged in to the updated Self-Service Portal — thank you! No further action is needed, and you are ready to renew this October!

If you have not logged in to the Self-Service Portal since March, please do so before October 15 by following these steps:

  • Go to https://selfservice.cdho.org
  • Click “Forgot your password?”
  • Enter the email address you have on file with CDHO
    • If you do not know the email on file or no longer have access to it, please email registration@cdho.org for assistance. Please do not sign into the Portal with a new email address – this is only for new applicants.
  • Click “Send verification code”
  • Enter the verification code sent to your email address and click “Verify Code”
  • Click “Continue”
  • Select a new password and enter it into “New Password” and “Confirm New Password”
  • Click “Continue”

Reminder: Collection of Demographic Information

Every year at renewal, RDHs are given the option to respond to questions that collect information on Indigenous identity, visible minority identity, sexual orientation and gender identity, and disability. We collect this data to better understand the demographics of the profession. CDHO is also working to improve equity and inclusion, and this data will help inform these initiatives. Summary data may be used for reports and analyses. We encourage you to complete these questions to help ensure we’re collecting accurate demographic information about the profession.
