To request a certificate of professional conduct, please complete Section 1 of the Professional Conduct Form and submit it via email to registration@cdho.org.

The normal processing time for this request is 15 business days and the fee is $15 per request, payable online through the Self-Service Portal.

If a regulatory body or educational institution uses another form, you will need to submit that and the CDHO form for completion. Ensure that the contact information for sending back the completed form is accurate since we do not verify this information on our end.
